Our commitment to community health
At Mayo Clinic Health System, we recognize that health and well‑being are shaped not only by medical care, but also by the environments where people live, work and connect. As an active partner in building vibrant, healthy communities, our commitment to patients, staff and neighbors extends beyond our clinic and hospital walls.
Through strong partnerships with local organizations, we support programs, initiatives and events that create lasting, positive impacts on community well‑being. Together, we can build healthier, more resilient hometowns.
We support community health by providing financial and in‑kind assistance to nonprofit organizations whose work aligns with our mission and priorities.
Eligibility and alignment
Nonprofit organizations located within the Mayo Clinic Health System service area are invited to apply for funding. Priority is given to organizations operating in communities where Mayo Clinic Health System has a physical presence. Requests must align with Mayo Clinic mission and values. We’re especially interested in efforts that align with our most recent Community Health Needs Assessment, listed below, and address the priorities that matter most to our neighbors.
A completed W‑9 form is required for all applications.
Community Health Needs Assessment (CHNA) priorities
Our CHNA priorities are identified through a comprehensive assessment conducted every three years. These priorities guide our community investments and partnerships.
Priority areas:
- Access to care
- Basic needs that influence health
- Chronic disease and contributing factors
- Mental health
- Substance misuse
Funding opportunities
Eligible nonprofit organizations may receive one community grant (Impact Grant or Spark Grant) and one community sponsorship per fiscal year. Available funding is limited, and not all requests can be funded; applications are reviewed competitively. All awarded funds must be used within one year of receipt.
For awards exceeding $5,000, an outcomes report is required upon project completion. A reporting template will be provided.
Mayo Clinic Health System grants and sponsorship

Impact Grant ($10,000–$30,000)
These competitive grants support programs, projects, or supplies that improve health and well‑being across the Mayo Clinic Health System service area.
Eligibility
Requests must:
- Align with CHNA priorities, or
- Clearly demonstrate a meaningful impact on community health and well‑being
Annual Grant Cycle
- Application period: September 1–30
- Review period: October 1–31
- Funding decisions announced: November 1
Apply: Application will be available on September 1

Spark Grant (Up to $9,999)
These grants support small‑scale programs, projects, or supplies that advance community health. Applications are reviewed quarterly.
Eligibility
Requests must:
- Align with CHNA priorities, or
- Clearly demonstrate a meaningful impact on community health and well‑being

Mayo Clinic Health System Sponsorship
Sponsorships support community events and fundraisers that strengthen local connections and align with Mayo Clinic Health System’s mission and CHNA priorities.
While sponsorships are an important way to support community engagement, a significant portion of our community investment budget is intentionally directed toward programming through community grants.
Please allow up to eight weeks from the submission date for sponsorship review and approval.
Requests we are unable to support
To ensure mission alignment and equitable use of funds, Mayo Clinic Health System is unable to support requests from or for:
- For‑profit organizations
- Labor or fraternal organizations
- Organizations whose mission or activities do not align with Mayo Clinic Health System values
- Political campaigns, candidates, parties, or partisan activities
- Religious organizations for theological purposes
- Youth or adult sports leagues or associations
- Replacement of funds already allocated or awarded
- Requests benefiting a single individual
- Entry fees or pledges
- Duplicate services already available in the community
- Reimbursement for expenses already incurred
- Travel expenses or registration fees
- Organizations with little or no presence in our service area
- Programs that limit participation based on race, age, religion, national origin, marital status, color, creed, sexual orientation, gender identity or expression, disability, genetic information, veteran status, or public assistance status
