    Mayo Post Acute Care

    You may benefit from Mayo Post Acute Care if you are recovering from an illness or surgery and no longer need acute care, but can't yet return home.

    While you are in Mayo Post Acute Care, you receive daily skilled care in a Mayo Clinic Health System hospital.

    If your condition permits, you will be encouraged to:
    • Participate in daily activities.
    • Perform your daily cares.
    • Dress in your own clothes.

    Creating your care plan

    You and your family will work with a team of caregivers to develop your individual care plan, focusing on meeting your physical, spiritual, and social needs. Your health care team will meet with you weekly to ensure you are progressing toward your planned goals.

    A team approach

    Your multidisciplinary team caring for you during your hospital stay can include:
    • Certified nursing assistants
    • Chaplains
    • Dietitians
    • Licensed practical nurses
    • Nurse practitioners
    • Pharmacists
    • Physicians

    Mayo Post Acute Care teams consult with providers from referring acute care hospitals, including other Mayo locations, should questions or concerns arise.
