    Fall Hometown Health Grant Opportunity

    Mayo Clinic Health System provides funding to support programs and initiatives designed to improve the health and well-being of the communities it serves.

    Through the Hometown Health Grant, Mayo Clinic Health System collaborates with local nonprofit organizations and community stakeholders to promote sustainable, community-driven approaches to enhancing health.

    Applications for the grant will be accepted from September 29 to October 20, 2025. Grants must be used within one year of receiving funding. After the project is complete, an outcomes report is required for grants over $5,000. A report template will be provided. Recipients will be notified by December 1, 2025, by email.

    Eligibility requirements

    Priority will be given to organizations operating in communities where Mayo Clinic Health System has a physical presence and to those requests that align with Mayo Clinic Health System Community Health Needs Assessment and strategic priorities.

    • Those CHNA priorities include:
      • Addressing social determinants of health, including food insecurity, housing, personal safety and transportation
      • Improving mental health with an emphasis on strengthening community mental health services and suicide prevention, and supporting resiliency across the lifespan
    • A W-9 must be submitted with all applications

    We are unable to support requests from or for:

    • For-profit organizations
    • Labor or fraternal groups
    • Organizations whose philosophy or mission contradicts or does not align with the mission or values of Mayo Clinic Health System
    • Political campaigns, candidates, parties or partisan activities
    • Religious organizations for support of theological functions
    • Youth and adult sports leagues
    • Replacing funds already allocated or available in funded grants
    • Sole benefit of an individual
    • Entry fees or pledges
    • Projects that are a duplication of an existing service
    • Reimbursement of expenditures already incurred
    • Assistance with travel expenses and registration fees
    • Organizations with little or no presence in a Mayo Clinic Health System service area
    • Budget shortfalls
    • Programs and projects that limit participation for reasons of race, age, religion, national origin, marital status, color, creed, sexual orientation, gender identity and expression, disability (physical and intellectual), genetic, veteran status and status with regard to public assistance
