To carry out the work, we collaborate with community stakeholders, develop and implement programs, and participate in activities that improve community health and address the social determinants of health. This is done through addressing Community Health Needs Assessment priorities; giving back to the community through financial and in-kind donations; providing volunteers at community events and educational presentations; collaborating with partners through coalitions, boards and special projects; and connecting the community with resources for general health and wellness.
