To carry out this work in Southeast Minnesota, we collaborate with community stakeholders, develop and implement programs, and participate in activities that improve community health, and address the social determinants of health. This is done through the Community Health Needs Assessment; giving to the community through financial and in-kind donations; providing volunteers at community events and medical experts for educational presentations; collaborating with community partners through coalitions, board service and special projects; and connecting the community with resources for general health and wellness.
