      How Mayo Clinic Advanced Care at Home Works

      Verified insurance coverage

      Mayo Clinic Health System will work with your insurer on coverage for care provided. Inpatient and outpatient insurance co-payments, co-insurance and deductibles may apply.

      Care led by Mayo Clinic providers

      Your care team is led by Mayo Clinic Health System physicians, and includes physicians, nurse practitioners, physician assistants, nurses, and a team of health care and service providers. You will have access to your care team 24/7 anytime day or night. Your care also may include in-home visits by a nurse practitioner or physician assistant, or other health care professionals, as determined by your care team.

      Technology delivered and set up in your home

      During your care, you will be provided with a computer tablet for video visits with your Mayo Clinic Health System care team, a telephone that connects directly to your care team, a personal emergency response bracelet, vital sign monitoring devices, a router for internet access and a backup power supply. Lab tests; mobile imaging, like ultrasounds and X-rays; and IV therapies also can be performed in your home.

      Seamless care transitions

      Your Mayo Clinic Health System care team will ensure your admittance to the program will be a seamless transition, with all your needs taken care of at home.

      Here's what a typical day at home looks like:

      • 7 a.m. — Community paramedic visit, blood draw for daily labs and virtual nurse check in
      • 8 a.m. — Advanced practice provider visit and virtual physician visit
      • 10 a.m. to noon — Children stop by to visit
      • Noon — X-ray
      • 2 p.m. — Shower with the help of an aide and virtual nurse check in
      • 3 p.m. — Physical therapy visit
      • 5 p.m. — Dinner provided by the hospital
      • 6 p.m. — In-person nurse visit, IV infusion and virtual nurse check in
      • 8 p.m. — Fall asleep in your own bed

      Safety is the priority

      The program has a thorough clinical approach to ensure your safety.

      Protections in place include:

      • The ability to reach your clinical team anytime day or night.
      • A bracelet you wear that will dispatch 911 emergency services when activated.
      • Monitoring equipment that reports information to your care team, such as blood pressure and heart rate.

      Leaving home during care

      During care, just as if you were in the hospital, you are asked to stay home. The care team will consider your diagnosis and circumstances should you need to leave home during your care.

      Changing your mind

      At any time, you can ask to be discharged from the program, and Mayo Clinic Health System will transition your care.

      Program discharge

      When you are well enough to be discharged, your Mayo Clinic Health System care team will ensure a smooth transition out of the program and the removal of all the home care technology.

      Determine qualification

      Your provider will evaluate several factors, including your diagnosis, available support system at home, proximity to the hospital and insurance coverage.
